For Chief Executive Officers and Nursing Leaders, RIVIAM provides a Hospital to Home service to fast-track patient discharge that is proven to save time and enhance patient care.

At the event, our focus is on this digital service that connects hospital teams with multiple community, housing and voluntary sector services providing a referral process that is quick and seamless and a Care Control Dashboard for easier decision making.

In 2024, RIVIAM managed over 2m patient referrals and sent over 3.4m emails and SMS messages on behalf of the NHS across England.
