We are seeking to recruit a community nurse to join the ECMHT OP in Dudley. The post is 37.5 Hours a week. Applicants will work within a care pathways approach (CPA) to coordinate care for older adults with a variety of complex Mental Health issues. Candidates are expected to demonstrate effective communication and computer skills to enable accurate and contemporaneous record keeping. Ability to conduct care and risk assessments along with person centred care planning and outcomes based care review is essential. Working with other member of the Multi-Disciplinary Team applicants are expected to effectively support and safeguard people with differing levels of capacity to promote service user choice and work to maximise independence through a model of recovery. Effective time management and interpersonal skills are considered essential as the post holder will need to liaise with GP practices and other supportive and partnership community resources. Main duties of the job: To work as a member of a multi-disciplinary Enhanced Community Mental Health Team to provide community psychiatric nursing care to people with severe mental health needs in the context of the care programme approach in secondary care. As a qualified nurse, the post holder will carry a caseload provide care co-ordination, implement and evaluation of care for people with mental health problems in the community. They will work in partnership with the multi-disciplinary team and contribute to the continuing development of the service.
