Our Trust is currently merging with Yeovil District Hospital to become one Trust covering the whole of Somerset.

12 Community Hospitals
Our 12 community inpatient wards provide specialist services across Somerset for adult patients who need 24-hour inpatient care for a combination of medical and physical needs.
Bridgwater Hospital, Burnham-On-Sea Hospital, Crewkerne Hospital, Chard Hospital, Frome Hospital, Minehead Hospital, Shepton Mallet Hospital, South Petherton Hospital, Wellington Hospital, West Mendip Hospital (Glastonbury), Williton Hospital, Wincanton Hospital.
Rapid Response / Discharge to Assess
Discharge to Assess (which you may hear referred to as D2A) has been created to help people get home more quickly, offering a range of services including assessment (measuring your health and ability to perform everyday tasks) and rehabilitation (helping you get back to your normal life). emailed Tamsin for document
MIU (Minor Injury Units)
We have MIUs in community hospitals based at Frome, Glastonbury, Shepton Mallet, Chard, Bridgwater, Minehead and Burnham-on-Sea. They are all run by emergency nurse practitioners, who are nurses with specialised training and skills in emergency and urgent care.
District Nursing
Our District Nursing is split into geographical neighbourhoods: North Sedgemoor, Minehead, Taunton, South Somerset, Mendips. The main focus is to provide nursing care to housebound patients in the home or residential care environment. Our district nursing service offers planned, individualised, patient centred care.
Mental Health
On Thursday 18 November the Health Service Journal held its annual ‘HSJ Awards’ ceremony in London, where we were honoured to be awarded Mental Health Trust of the Year, having been shortlisted for the second year running.
Our trust provides mental health services to all age ranges and supports people with a range of mental health problems whether they are mild, moderate, or severe. It’s what makes the Trust an exciting and dynamic environment in which to work and build a progressive career.
Our Preceptorship programme:
Preceptorship is a period to guide and support all newly qualified practitioners in the transition from student to autonomous practitioner.
We offer a fully integrated multi-disciplinary preceptorship programme.
• Open to all newly qualified staff
• Separated into dual stranded approach where preceptees come together for development sessions while the clinical competencies are delivered on the ground within your clinical team
• Offering a virtual delivery to ensure maximum flexibility and accessibility for colleagues and teams
• Looking to align the programme across the whole Somerset system.
