Are you passionate about improving the quality of services for people living with learning disabilities and/or autism?
Are you a qualified Health or Social Care Practitioner?
Do you have significant experience of working at a senior level in Learning Disability and/or Autism Services?
Are you knowledgeable about the Transforming Care Agenda and CTR/CETR Policy and guidance?
Do you have experience in project management and a track record of successful service improvement?

As part of delivering the Transforming Care Agenda, Midlands Partnership Foundation Trust (MPFT) are leading on a project to roll out an all-age Dynamic Support Register (DSR) for people with learning disabilities and or autism who are at the highest risk of a hospital admission. You will drive forward the work required to develop the register; establish robust processes, procedures and governance arrangements to support its use; publicise and promote its aims and benefits; and successfully introduce and embed it across South Staffordshire. You will link with all relevant local partner organisations including Local Authorities, Health Trusts, CCGs and Education. You will work alongside a second DSR Project Lead who will be working on the same project in the north of the County. You will ensure the work required to deliver on the project is co-produced with experts by experience.
