Band 5, £28,407 to £34,581 a year per annum / pro rata An exciting opportunity has arisen within the expansion of the children's crisis and home treatment team where we have a number of practitioner roles available. We are looking for an enthusiastic and motivated registered professional to join our friendly multi-agency team and help us on our journey to offering outstanding care for the children and young people (CYP) within Coventry and Warwickshire. The Crisis and home treatment model is part of the Tier 3.5 service provision which focuses on a high risk management pathway, to prevent avoidable admissions and support discharges from inpatient units. This is so CYP are in the least restrictive service for their needs. The team has been going through a transformation from the role of the Acute Liaison team to be embedded into the Crisis and Home treatment model for CYP and providing a seamless crisis response no matter where the access point is.
