Central London Community Healthcare NHS Trust (CLCH) operates in 11 London boroughs (as shown below) and Hertfordshire, providing community health services and care to more than two million people.
We support our patients at every stage of their lives, providing health visiting for new-born babies through to community nursing, specialist long term care support and palliative care for people towards the end of their lives.
Our vision is to deliver great care closer to home and our mission is to work together to give children a better start and adults greater independence.
